Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

You can enter in the practice and you live in the neighbourhood and no general practitioner have in the neighbourhood. The following postal area codes are available: 1311, 1312, 1313, 1314, 1315, 1316, 1317, 1319, 1323, 1324.
If you do have a general practitioner, but want to switch, that is certainly possible, but I would like you to explain why you want to switch from general practitioner and you will be invited for an introductory meeting.
Registration is done by filling in and sending the form below. The information will then be sent to our administration. When this has been checked, you will receive a confirmation of registration.
Practical Information
We believe it is important that we can help you well and that you are satisfied with our care. Despite our good intentions and efforts, it may happen that you feel that we have approached or treated you carelessly or incorrectly. We would like to hear from you when you are not satisfied and invite you to discuss this with us. Make an appointment with us to clarify the problem. This usually solves a lot. If we cannot jointly arrive at a solution that is satisfactory to you, you can submit a written complaint to the independent complaints committee of the SKGE. The complaints procedure is free of charge for you.
